Data Deletion Request

To request deletion of your EllisMD account and associated personal information, email support@ellismd.health with the subject “Data Deletion Request.”

EllisMD will verify the identity of the requester before processing the request. Information will be deleted or de-identified as appropriate, except for records EllisMD is required or permitted to retain for medical-record retention, legal, regulatory, billing, fraud-prevention, or security purposes.
